Neck Pain in Cary

You turn your whole body to check the blind spot. The tension between your shoulder blades never fully lets go. Stiffness has become your normal, and normal is not the same as fine.

01What Causes It

What causes neck pain?

You wake up stiff. You sleep on the wrong pillow and pay for it for three days. By the end of a screen-heavy afternoon the tension has spread across both shoulders. Your neck holds your head up all day and still has to turn, tilt and look up, while protecting the spinal cord running through it. When the joints stiffen or the muscles are overloaded, pain and lost motion follow. Left alone, they tend to bring company: numbness down the arm, chronic headaches, and weakness through the shoulders and upper back.

Hours at a monitor or looking down at a phone hold your head forward of where it belongs, and the muscles at the base of your skull spend the whole day holding it there. Ergonomics that put the screen too low or the keyboard too far away make it worse. It is never one afternoon that does the damage. It is the same position, repeated, until your neck has less range than it used to.

Neck muscles strain in two ways: slowly, from repeating one movement or holding one position, and suddenly, from an impact or a wrench that asks them to protect the spine faster than they can. Both leave the muscles guarded and tight. The pain is real, and so is the stiffness that follows it, but muscle strain responds quickly to hands-on treatment and graded movement.

The joints of the cervical spine stiffen with arthritis and with normal aging. A stiff joint moves less, which means the joints and muscles around it have to work harder, which is why the stiffness tends to spread rather than stay put. Restoring whatever motion is available in those segments, and then keeping it, is more useful than waiting for the joint to settle on its own.

When a herniated disc or a poorly aligned vertebra narrows the space a nerve root needs, the nerve gets compressed. The symptoms then show up away from your neck: pain radiating into the shoulder or down the arm, numbness, tingling, or a hand that has lost its grip. Those signs deserve a proper neurological screen, because nerve involvement changes both the plan and the timeline.

Stress lands in the neck and shoulders. The muscles across the top of your shoulders sit slightly contracted all day, and slightly contracted all day is enough to cause pain and to pull on the joints underneath. It is also why neck tension so often turns into a chronic headache. Releasing that tension, then rebuilding endurance in the muscles that should be holding you up, breaks the cycle.

Seven hours in a position your neck dislikes is a long exposure. Stomach sleeping keeps the neck rotated the entire night. A pillow that is too high or too flat leaves the cervical spine unsupported. Sleeping on your back with support that keeps the neck in line with the rest of your spine is the simplest change available, and it often changes how the mornings feel.

02How PT Helps

How physical therapy in Cary helps.

Neck pain left to run its own course tends to cost you more than comfort: motion, arm strength, and a headache habit that follows you through the day. Physical therapy starts by finding the source instead of chasing the ache. Range of motion testing shows what is restricted, palpation finds the stiff or irritated segments, a neurological screen checks whether a nerve is involved, and a look at your posture and muscle endurance shows what your daily habits are doing to you. North Carolina has unrestricted direct access to physical therapy, so no physician referral is needed for that evaluation. You get Dr. Heather Smith, PT, DPT, OCS, a board-certified orthopedic clinical specialist, for the whole 30 to 60 minutes, one on one, every visit. Then four phases. Pain: manual therapy to restore joint movement and ease the tension. Prime: targeted stretching and postural retraining to win back motion and alignment. Perform: strengthening that stabilizes the cervical spine through a real day. Prevent: the ergonomic and sleeping changes that stop it returning. Most patients come 2 to 3 times a week for 4 to 6 weeks. Your coverage is verified before treatment begins.

03What To Expect

What to expect from your recovery.

01

Evaluation

Heather measures how far your neck moves in each direction, palpates the joints for stiffness and irritation, screens for nerve involvement, and assesses your posture and muscle endurance. If your symptoms point somewhere that needs a physician, you will be told. Otherwise you leave with the source named and a timeline.

02

Hands-on relief and mobility

Manual therapy restores movement in the stiff cervical segments and eases the muscle tension pulling on them, and targeted stretching returns the flexibility you have lost. You get a short set of movements to keep doing between visits, because what happens on the other six days matters just as much.

03

Strength, posture and prevention

As the pain settles we strengthen the muscles that stabilize your neck and upper back, and retrain the posture that kept overloading them. We work through your desk setup, your screen height and your sleeping position, so your neck holds its gains once therapy ends.
