You did not come in to fill out forms. You came in because something hurts and you want to know what is actually going on. So here is how the paperwork side works before you arrive, what to have with you, and how much time to leave for your first appointment. Read it once and the admin part is handled.
Once you request an appointment, Grace Overton calls to set your time and sends your intake paperwork over so you can complete it before you arrive. It covers your history, what you are feeling now, and your insurance details. If you would rather fill it in at the clinic instead, say so on that call and we will have it ready for you.
Your insurance card and a photo ID. Anything you already hold about the problem itself: x-rays, imaging reports, notes from your physician, a surgical report if you have had surgery. And comfortable clothing you can move easily in, because Dr. Heather Smith needs to watch the area actually move before she can tell you much about it.
Plan on 30 to 60 minutes for the visit itself, plus a few minutes at the front if you are finishing your paperwork here. That whole session is one on one with Dr. Smith, so the time you set aside goes into your evaluation and your treatment rather than into sitting and waiting for someone to be free.
Not sure whether you need to bring something? Call (919) 694-6186 before your visit and ask. And no physician referral is needed to be seen in North Carolina.
